Brookings CORE Response strives to expand access to healthcare services for every community member in Curry County. This means we work with everyone from doctors and specialists to state benefit workers and your health insurance company. You can read about each program below to understand who we serve and how to access them.
Resource Navigation
Resource Navigation is a case management program designed to help people navigate finding, applying for, and accessing different types of benefits. We offer assistance in obtaining the following resources for individuals who are without housing or at risk of losing their housing, and who are considered low income:
- Healthcare services: Oregon Health Plan, primary care, medical transportation
- Food resources: SNAP (food stamps)
- Identification: ID/license, social security card, or birth certificate
- Income: employment, disability, TANF, Social Security
- Behavioral health: peer support, mental health provider, or addiction treatment
We are able to provide some financial assistance on a case-by-case basis. This program does not offer gas or motel vouchers. To see if you qualify, please call (541) 251-0825.
Medical Respite – COMING SOON
Medical Respite is short-term residential care that offers people experiencing homelessness a safe place to recover while accessing medical care and other supportive services.
Many people experiencing homelessness grapple with chronic health conditions. Compounded by the lack of regular healthcare access, this leads to frequent emergency department visits and inpatient stays to address acute care incidents. Hospitalization ends in discharges to the street for individuals who lack safe, stable housing. When this happens, quality of life suffers and the individual’s health can often deteriorate quickly, leading to readmittance to the hospital. Medical respite programs can bridge this healthcare gap and keep people from getting worse, as well as connecting them to long term services.
This program is offered through hospital inpatient services and does not accept outside referrals.
Hospital Consultation – COMING SOON
Along with Medical Respite, we provide Hospital Consultation for hospitals and healthcare providers who are struggling to address a community member’s health conditions due to a social need, such as food, housing, income, or other resources.
Our role is to provide on-the-ground, responsive and appropriate connection to resources needed for community members before they leave the hospital, as well as follow up support. For providers, we know that keeping up with the changing resources in a community is a full time job, so we have dedicated support to assist with a referral, a warm hand-off, or a team meeting for consultation.
