Resource Navigation is a peer-delivered, person-centered case management program led by our dedicated Community Health Workers who have lived experience and understand the challenges of navigating social services systems. These professionals serve as your advocates, liaisons, and sources of support on your journey to improved health and overall well-being. Our goal is to help you overcome barriers and connect you to vital resources that can make a significant difference in your life.
How Resource Navigation Can Help You
Our program is tailored to meet your unique needs, and our Community Health Workers are here to assist you in the following areas:
- Income Support: We can help you access government benefits, explore employment opportunities, and connect with other income-related resources to improve your financial stability.
- Healthcare Access: We guide you through the process of enrolling in the Oregon Health Plan (Medicaid health insurance), as well as connecting you to Primary Care, Specialty Care (as referred by your provider), dental care, mental health services, or treatment for addiction.
- Legal Assistance: If you require legal support, our team can connect you to the necessary resources and services, and even make a referral on your behalf.
- Identification: We assist in obtaining crucial identification documents, such as ID/Driver’s License, Social Security card, and birth certificates.
- Transportation: Our program offers support for transportation needs, whether through covered transportation benefits via insurance, public transportation options, or internal resources.
- Food Resources: We connect you to food resources, including food bank referrals, information about daily meal locations, and access to internal food resources.
- Other Resource Support: If you have specific needs or face other challenges, our Community Health Workers are here to find solutions and support you on your journey to stability.
This program is for community members who are without stable housing and social support, as well as those who have no access to shelter or housing. Eligibility is determined through an initial assessment in our office or over the phone.
